ABSTRACT

Objective:To evaluate multi-channel transcatheter embolotherapy for type Ⅱ endoleak originating from lumbar arteries after endovascular abdominal aortic aneurysm repair (EVAR).Methods:Data of 8 cases of type Ⅱ endoleak after EVAR from Oct 2017 to Nov 2020 at the Department of Vascular and Endovascular Surgery, Henan Provincial People's Hospital were retrospectively analyzed.Results:All patients who suffered from type Ⅱ endoleak that originated from lumbar arteries after EVAR were successfully treated with coils and mixture of Compant medical glue and iodipin through multi-channel. The technical success rate was 100%, the operative time was 80-150 min. Right lower limb dyskinesia occurred in 1 patient after operation, the symptom disappeared by anticoagulation and trophic neurotherapy for 2 months. Type Ⅱ endoleak didn't recur in all patients, and no mortality during the 4-38(14.1) months follow-up period.Conclusion:Multi-channel transcatheter embolotherapy has definite effects for the treatment of type Ⅱ endoleak from lumbar arteries after EVAR, with high technical feasibility, few perioperative complications, low mortality among other advantages. The results of short and medium term are satisfactory.

ABSTRACT

Objective:To explore the feasibility, safety and effectivity of applying transmesenteric vein extrahepatic portosystemic shunt (TEPS) to treat extrahepatic portal vein obstructive disease (EHPVOD).Methods:From December 2020 to April 2021, 12 patients with EHPVOD in the Vascular Surgery Department of Zhengzhou University People′s Hospital were prospectively enrolled in the study. The infra-umbilical median longitudinal minilaparotomy was performed to expose the branch of superior mesenteric vein (SMV). RUPS-100 was introduced into the trunk of SMV. A balloon with a diameter of 20 mm was introduced through right internal jugular vein (RIJV) into inferior vena cava (IVC). Under fluoroscopy, RUPS-100 was used to puncture the balloon in IVC. A stiff guide wire was used to establish the pathway between RIJV and SMV. Finally the portosystemic shunt between IVC and SMV was established with a covered stent-graft. The total operative time, the time of establishing portosystemic shunt alone, the dosage of contrast agent, the preoperative and postoperative pressure of SMV were recorded. Paired t test was used to compare the preoperative and postoperative pressure of SMV. Results:All 12 patients were successfully performed TEPS. The total operative time was (113±32) min, the time of establishing portosystemic shunt alone was (31±5) min, the dosage of contrast agent was (129±48) ml. The postoperative pressure of SMV [(14.3±2.1) mmHg] decreased significantly ( t=20.125, P<0.01) compared to baseline [(27.8±2.7) mmHg]. All portal hypertension symptoms released after the operations.There was 1 case of delayed incision healing, 1 case of bacteremia and 1 case of slight hepatic encephalopathy, but all of them were cured. There was no death case. Postoperative CT showed all portosystemic shunts were patent. Conclusion:TEPS is a new, safe, effective and feasible treatment method for patients of acute and chronic EHPVOD.

ABSTRACT

BACKGROUND:Primates are considered to be the most appropriate animal model of lumbar intervertebraldisc degeneration, but the disc degenerated characteristics of monkeys were rarely reported. OBJECTIVE:To verify the degenerated regularity and characteristics of lumbar intervertebral disks in rhesus monkeys with magnetic resonance T2 mapping and T1ρimaging technology. METHODS:The sagittal lumbar intervertebral disc magnetic resonance T2 weighted imaging,T2 weighted mapping imaging and T1ρweighted imaging of 63 adult rhesus monkeys were acquired on 1.5T magnetic resonance equipment. The T2-map value and T1ρvalue of lumbar intervertebral disc regions of interest were calculated on the post-processing workstation. RESULTS AND CONCLUSION:(1) This study obtained 425 better magnetic resonance images of lumbar intervertebral disks in adult rhesus monkeys. T2-map value and T1ρvalue of nucleus pulposus were most consistent by different persons, and the Kappa coefficient was more than 0.93. (2) The T2-map value and T1ρvalue of nucleus pulposus were both negatively correlated significantly with Pfirrmann grades (r=-0.842, P<0.01;r=-0.896, P<0.01). The T1ρvalue and T2-map value of nucleus pulposus were significantly statistical y different between Pfirrmann grades I-IV (P<0.001, P<0.001). The T1ρvalue of nucleus pulposus was negatively correlated significantly with Pfirrmann grade II-III (r=-0.517, P<0.01) and Pfirrmann grade IV-V (r=-0.499, P<0.01). The T2-map value of nucleus pulposus was also negatively correlated significantly with Pfirrmann grade II-III (r=-0.617, P<0.01) and Pfirrmann grade IV-V (r=-0.652, P<0.01). (3) The T2-map value of L1-2 and L2-3 segments nucleus pulposus were significantly lower than that in L6-7 and L7-S1 segments (P<0.05). (4) There were significant differences in age among the T1ρvalue and T2-map value of nucleus pulposus (r=-0.702, P<0.001, r=-0.730, P<0.001). (5) It is concluded that magnetic resonance T2 mapping and T1ρimaging technology can objectively and sensitively assess the degenerated process of nucleus pulposus in rhesus monkeys. The degeneration in upper lumbar segments (L1-2 and L2-3) was earlier and more severe than that in lower lumbar segments (L6-7 and L7-S1) in rhesus monkeys. Age is one of the most important factors in lumbar intervertebral disc degeneration of adult rhesus monkeys.

ABSTRACT

BACKGROUND:There are many animal models used for studying discogenic pain, but percutaneous puncture annulus is rarely reported. Minimal y invasive approach to establish a discogenic pain model in Spraque-Dawley rats could reduce the interference factors of surgical trauma. OBJECTIVE:To establish a Spraque-Dawley rat model of discogenic pain by percutaneous puncture annulus, with easy operations, high stability and obtaining large-scale productions, and to confirm the model by the results of behavior, MRI and molecular biology. METHODS:Eighty-eight male Sprague-Dawley rats, of specific pathogen free level, were randomly divided into three groups, model group (n=44), control group (n=10) and sham group (n=34). In the model group, the annulus was percutaneously punctured under X-ray guidance;while rats in the sham group were punctured at the paravertebral tissue, rather than the annulus. RESULTS AND CONCLUSION:The 50%mechanical withdrawal threshold of both hind paws in model group were reduced compared with control group and sham group. In the model group, the L 5/6 intervertebral disc degeneration was apparently visible, and the degree of degeneration was aggravated along the time. In the model group, the expression of calcitonin gene-related peptide in dorsal root ganglion of rats began to increase at 3 days post-operation and reached the peak at 21 days post-operation, then remained at high levels until the 35th day post-operation. The expression of tumor necrosis factor-αin dorsal root ganglion of rats increased at 3 days post-operation and reached the peak at 14 days post-operation, then remained at a higher level until the 35th day post-operation. The experiment result verifies that the discogenic pain model of Spraque-Dawley rats induced by percutaneous puncture annulus has the advantages of good stability and less trauma. The model can be used to study discogenic pain.

OBJECTIVE@#To study the expression of phosphorylated extracellular regulator kinase (ERK1/2) on nasal polyps.@*METHOD@#The immunohistochemistry and Western-blot were used to study the phosphorylation of ERK1/2 on the normal nasal mucosa and nasal polyps.@*RESULT@#The phosphorylated ERK1/2 was mainly expressed on the hyperplastic epithelium, inflammatory cells, glandular epithelium and endothelium of nasal polyps by immunohistochemistry, which was much stronger on nasal polyps than that of normal mucosa. The expression of phosphorylated ERK1/2 was 4.2 times than that of normal mucosa.@*CONCLUSION@#The study indicated that the phosphorylation of ERK1/2 might play an important role on the occurrence and progression of nasal polyps.

OBJECTIVE@#To study the expression of MMP-9 and eosinophils in nasal polyps and its influence on the prognosis.@*METHOD@#Study the expression of MMP-9 and EOS on the tissue of nasal polyps of patients who were cured or recurrent and normal nasal mucosa (NM) by immunohistochemistry and HE staining.@*RESULT@#In the patients of recurrent group (Group B), the expression of MMP-9 was much stronger than that of good prognosis group (Group A) and NM. The MMP-9 was stronger in the eosinophil nasal polyps than that of non-eosinophil groups. The percentage of EOS in Group A was 16.67%, while 62.50% in the Group B.@*CONCLUSION@#The stronger expression of MMP-9 on Group B and eosinophil group indicated that MMP-9 might affect the prognosis of nasal polyps through inducing the number of EOS in the mucosa. MMP-9 may be one of the risk factors for the relapse of nasal polyps.
